Privacy Notice

Your Data and Trillium Osteopathy

You may have seen a lot in the media lately regarding data changes coming into effect as of May 25th, 2018.  The following is an abbreviation of the full Data Policy as set out by the new GDPR law which comes into effect on this date.

This notice pertains to Trillium Osteopathy, c/o Crossroads Clinic, 82 High Street, Gorseinon, Swansea, SA4 4BL, providing Osteopathy, Dry Needling and Kinesio Taping.

When you provide your data to Trillium Osteopathy, it is processed and stored for the following reasons:

We need to collect relevant data about your health, lifestyle, relevant history and so forth to ensure you receive the best treatment possible for you as an individual.  It is your right to refuse to provide information, however if so, we would not be able to provide treatment.

We have a legitimate interest to collect all relevant information, in order to effectively and safely treat you the patient.  We may need to contact you to confirm or arrange appointments, or provide relevant updates regarding your care.  This is also ‘legitimate interest’.  With your consent, we may send you general health information in the form of articles or advice.  It is your right to withdraw your consent and we will be happy to do so – please discuss this with your Osteopath and all necessary updates will be made on your behalf.

Legally, we must retain your records for a period of 8 years after your very last appointment – if the patient is a minor this would be up until the age of 25 minimum, dependant on the age of the patient.  After this time you may request that this information be safely destroyed or it shall be securely retained indefinitely.

Your data including your examination form, treatment records and all other relevant data are safely stored on paper within a locked filing cabinet on premises.  Filing cabinet keys are safely and securely locked within a key safe.  The building is locked and there is a security alarm outside of business hours.  Computers are password protected, regularly backed up, with up to date security and virus scanning software.

At no time will your data be shared with individuals do not need access, without your written consent.  The following do have routine access:

Your practitioner(s) in order to provide you with treatment

Reception Staff at Crossroads Clinic who may take your name and contact details to forward to us in order to arrange appointments.  Please note that staff at crossroads clinic do not have access to the data that we hold – if you are an existing patient or client of Crossroads Clinic or any of the Individuals working from Crossroads, you will have separate files and data agreements in place with them.

You have the right to see what data we hold about you, and you may at any time ask us to amend any factual errors.  For further information please contact Rebecca Wedlake.

We want you to be completely confident that all data held is being appropriately stored, processed and used and that this information is only accessed by relevant individuals.  However, if you have concerns or complains, you can contract the Data Controller at Trillium Osteopathy Rebecca Wedlake, at or by calling 07935 225 458.  If you do not receive a satisfactory response to your concern, it is your right to raise this issue with the Information Commissioner’s Office.
